Individualized Supported Living

Living on one’s own is a major milestone that is filled with independent choices and a sense of pride and accomplishment. Often, individuals living with developmental disabilities are cared for by family members, friends, or in facilities but they may wish to live independently. When the time comes that the individual, family, friends, guardian, etc know that the individual is ready to live independently, Creative Pathways is available to help assist the individuals in finding their own housing and living successfully on their own. 

Individualized Supported Living (ISL) services provide individuals with developmental disabilities the opportunity to be supported in living independently. ISL services allow an individual to live in a home of their own, which may include living with up to two housemates. Each individual has their own space so they can make decisions about the atmosphere of their home. It is important that each individual is comfortable in their own home and have the opportunity to live in the least restrictive environment possible, while receiving the level of support they require. 

Staff are available up to 24 hours a day to assist individuals with whatever day-to-day tasks they may need. Each staff member is trained on the individuals’ Individualized Support Plan (ISP) which expresses the individuals goals, needs, and interests. Staff provide individuals support in a variety of areas which include in-home and community based supports. 

In-home supports may include but are not limited to: 

  • Budgeting and Money Management
  • Cooking and Nutrition
  • Wellness
  • Hygiene Assistance
  • Home Cleaning
  • Health and Safety
  • Medication Administration

Once individuals are living in their own homes and receiving our services, it is important that they feel that they are members of the community in which they live. As a part of our ISL services, our staff are trained to assist individuals in the community as well. 

Community supports may include but are not limited to:

  • Attending community events
  • Shopping
  • Engaging in groups or activities
  • Participating in trips
  • Outdoor activities
  • Visiting community facilities
  • Sport activities
  • Special Olympics

While the decision to live on one’s own is a major decision, even more so when the individual needs support to do so, Creative Pathways ensures that the individual will be able to live with increased independence while receiving the assistance they require. While individuals are living independently, it is important that they are still connected with their friends and family. That is why Creative Pathways highly recommends visits from friends and family, as well as encourages friends and family to participate in the various activities our individuals are engaged in. 

Creative Pathways ISL services ensure that each individual has the chance to live with independence, support, and success based on their own needs and interests. Host Home

Host Home is an arrangement in which an individual(s) with a disability choose to live with an individual, couple, or a family in the community to share their life experiences together. A host home could be a single person, a college student, single parents, empty nesters or a two-parent family with children or a person could live with an individual in their home, who wants to share their life with an individual with a disability. Host Home can be provided in the individual’s home (Companion Services) or in the home of the caregiver.

Host Homes provide benefits similar to ISL’s in community connection and integration, independent skill development, and care and support.
